We welcome new patients to our practice. Anyone who wishes to register with us needs to: 

• Live within our Practice Boundary (Postcode LE3 Leicester City Council and not Blaby County Council)
• Complete a registration form and health questionnaire which are available online via the links below.
• Bring in photographic proof of identification (if over 18) and proof of residence (e.g. a recent Council tax or gas or electricity bill)
• Immunisation history for those born outside the UK. 

Please note that the registration process can take up to five working days from when we receive the required forms and proof of identity and address.

We are also using a new NHS online service called Register with a GP surgery that makes it easy to register with this GP surgery.

Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number.

Please note that the registration process then normally takes up to 10 working days. 

Paper forms are still available if you need one.
