New Patient Registration

MerridaleWe welcome new patients to our practice. Anyone who wishes to register with us needs to: 

  • Live within our Practice Boundary (Postcode LE3 Leicester City Council and not Blaby County Council)
  • Complete a registration form which is available upon request from main reception or download one from the above "Registering Online" tab. 
  • Bring in their NHS medical card (if available) when wishing to register
  • Bring in photographic proof of identification (if over 18) and proof of residence (e.g. a recent Council tax or gas or electricity bill)

Please note that the registration process then normally takes up to 10 working days from when we receive the required forms and proof of identity and address. Once a new patient registration request has been processed an invitation will be sent to attend for a new patient check. This will usually be carried out by a healthcare assistant and will include an initial health assessment.
